The home care program provides in home care and support to member First Nations based on individual medical needs.
Considering each person’s needs, our team develops a holistic care plan with individuals and their families.
Our team includes three Residential Care Aids, doctors, physiotherapists and Community Health Nurses who work as an integrated team with CSFS Mobile Diabetes staff. We also work collaboratively with each band-funded homemaker program.
We mostly provide services to adults in the 30-59 and 60+ stages of the life cycle. However, we will provide our services to anyone in need at any age.
We currently provide Home Care services on-reserve to: Wet’suwet’en, Burns Lake Band, Nadleh Whut’en, Cheslatta Carrier Nation, Nee Tahi Buhn, Yekooche First Nation, Skin Tyee, Stellat’en, Takla Lake First Nation and Saik’uz First Nation.
We accept referrals from community members and professionals.
After receiving a referral, we complete an individualized assessment to determine specific needs and develop a care plan in collaboration with the client and their family.

Hours of operation: Home Care services are offered from Monday to Friday at various times.
> Services for palliative care and respite service varies on client need. The team consists of RCAs and a Registered Nurse that rotate throughout our member First Nations.
