Start a New Application
A simple starting point for anyone working with CSFS or Member Nations on research.
New Application (External Researchers)
For university partners, students and external organizations beginning a project with CSFS or Member Nations. This form confirms that your proposed project meets CSFS ethical, cultural and community-review requirements.
CSFS Staff Application
For CSFS employees undertaking research or evaluation projects connected to their program or service area. This form ensures staff-led studies move through the proper cultural, ethical and community-review pathways while reducing unnecessary duplication in the approval process.
Amendment or Renewal Application
Use this form when you need to:
- Make changes to an approved protocol
- Renew ethics approval
- Submit a renewal that includes amendments
This process ensures all updates continue to respect community expectations and uphold CSFS ethical standards throughout the project.
Study Closure Report
All external researchers and CSFS staff must submit a closure report within six months of a study’s end date.
Use this form if your project:
- has fully concluded
- did not proceed as planned
- was halted before completion
The closure report confirms all activities have ended and helps CSFS maintain clear, responsible research records.
