About the Program

Bringing diabetes care directly into communities helps remove barriers to testing, treatment and follow-up. During each clinic visit, patients meet one-on-one with specialized nurses who provide comprehensive diabetes screening and education in a supportive, culturally safe environment.

Appointments include foot and eye assessments, along with blood and urine testing to check the health of organs commonly affected by diabetes. After each visit, patients receive a copy of their results and a report is shared with their primary care provider to support continuity of care.

Care doesn’t stop when the clinic leaves. Ongoing follow-up and specialist support are provided through telehealth, helping patients stay connected to care between visits.

Who This Serves

You may want to be tested for diabetes if you notice:

Increased thirst
Frequent urination
Unexplained weight loss or weight gain
Ongoing fatigue
Blurry vision
Frequent infections
Cuts or bruises that heal slowly
Tingling, pain or numbness in the hands or feet

Regular testing is encouraged, especially for individuals with higher risk factors.

Members of CSFS Member Nations living with diabetes or pre-diabetes
Adults and children of any age who want screening when clinics are available
Community members interested in diabetes prevention and management
