About the Program

Home Care helps community members receive the medical and personal support they need while remaining in their own homes and communities. Each person’s needs are unique, so our team works with individuals and families to create a holistic, culturally grounded care plan.

Our team includes Residential Care Aides, Community Health Nurses, Mental Health Clinicians, physiotherapists and doctors who work closely with the CSFS Mobile Diabetes team and each Nation’s homemaker program. Together, we focus on supporting safety, wellness and dignity at home.

Eligibility

Eligibility is based on health needs and is determined through an assessment by a nurse or clinician.

Home Care services are currently offered on-reserve in:

  • Burns Lake Band
  • Nadleh Whut’en
  • Cheslatta Carrier Nation
  • Nee Tahi Buhn
  • Yekooche First Nation
  • Skin Tyee Nation
  • Stellat’en First Nation

Important to know

  • Home Care does not include homemaker services such as cooking, cleaning or firewood
  • Homemaker supports are provided through Band-funded homemaker programs
  • If you’re unsure whether you or a family member qualifies, the Home Care team encourages you to reach out. They can walk you through the process and help determine next steps

How to Access Support

You can access Home Care by:

  • Calling your local Home Care office
  • Being referred by a doctor, nurse, community health worker or family member
    • Once a referral is received, our team completes an individualized assessment and works with you to create your care plan.
