Take the CSFS community survey

We want to hear from you! Carrier Sekani Family Services (CSFS) is conducting a community survey on our programs and service delivery and we would love to learn from you.

The survey asks people in member Nations how they experience our organization and access CSFS services in their community. The survey is open until Friday, September 15, 2023 at 11:45 pm PT and takes approximately 15 minutes to complete.

For a chance to win one of three prepaid Visa cards ($250, $150 or $75), please provide your name and contact information at the end of the survey. You can help us by sharing the survey link with your friends, families and colleagues so we can hear from as many people as possible. If you know someone who may have difficulty accessing or using a computer, please support them with filling out the survey.

You can take the survey online on any tablet, computer or mobile device. Printed copies may also be available at your local CSFS office or health centre.
